Support for carers in Leeds Teaching Hospitals
Carers Leeds offers support to carers of older people on wards in Leeds Teaching Hospitals from their base at St James’s University Hospital.
Our Carer Support Worker Lorraine Lumb supports carers of people with dementia or memory problems.
Clare McNeill supports carers and families when their loved one is in hospital and will either leave to move into a care home, or will return home with a package of care. Clare specialises in the particular issues faced by people who pay for their care.
Clare also supports carers who are supporting someone at home and looking for care packages or care homes. If you’d like to refer a carer for support, you can do so here.
Both Lorraine and Clare focus on involving carers in discharge planning and ensuring that the carers are informed about services available to them in the community.

Hospital 2 Home (Support for carers)
Age UK Leeds offers help for older people who have had a short stay in St James’s Hospital to facilitate discharge, get settled in at home and prevent readmission.
Carers Leeds works in partnership with this scheme to ensure that carers identified within this project are offered support and are linked in with the Carers Leeds service.
Carers receive a visit at home from a Carer Support Worker who makes sure that the carer’s needs are recognised and that the carer is getting all the help they need to make the home situation safe and sustainable.
