Who we are:

 

Leeds Oak Alliance is a group of five charities, working together in partnership to help and support adults living with frailty and / or at the end of their life, and their carers.

Our focus is to help people to live and age well, and to provide the services that really matter to people approaching the end of their life in Leeds.

What we do:

 

Our information hub at The Atrium, Bexley Wing, St James’s University Hospital is open to the public, patients, carers and professionals offering information and support.

Our friendly experienced Leeds Oak Alliance team will talk through your circumstances and requirements, supporting and signposting you to the most appropriate service offered by the Leeds Oak Alliance organisations.
