We know its not always possible to get to Carers Leeds city centre office for information, advice and support with your caring role, so we have outreach Carer Support Workers who come to you.

This community carers clinic is based in Chapletown so if you are a carer and also a patient St Martins Practice, you can book an appointment to see a Carer Support Worker at this local GP surgery.

To make an appointment simply speak to reception staff in your GP Surgery, or call them on 0113 221 1888. Our Carer Support Worker Carrie will offer you a 45 minute appointment in St Martins Practice providing emotional support as well as practical support surrounding your caring role.  We help carers navigate through the system, in order to access appropriate services and provision, including welfare benefits.
