We cannot accept a referral unless you have the consent of the carer. By submitting a form you are confirming that you have obtained the carer’s consent to make this referral and share their details with Carers Leeds.
Carers Leeds accepts referrals from professionals and self-referrals from carers. We aim to respond to referrals from professionals within 5 working days. This is usually a phone call to the carer for an initial conversation, unless a carer would like to be contacted in another way.
