Leaving hospital can be a confusing and stressful time for patients and families. This Share and Learn session, delivered by Carers UK, will provide clear, practical guidance on how hospital discharge should work and what support should be available.

The session will cover:

  • Reablement Care and Intermediate Care

  • Hospital discharge rules and assessments

  • What to ask for before leaving hospital

  • The different assessments involved, including needs assessments, carers assessments and occupational therapy assessments

Whether you are a patient, a carer, or supporting someone through discharge, this session will help you understand your rights, your options, and how to ensure the right support is in place.
